Re-New Membership

To remain a member of the Laverton Community Integrated Services Inc., a person must;
Fill out this annual membership renewal form, and
Pay to Laverton Community Integrated Services an annual membership fee of $5.00[this fee goes towards administration costs of servicing members].
Annual membership runs from 1^stJuly to 30^th June.
To renew membership, the completed form and membership fee must be provided to the Laverton Community Integrated Services by 30^th September of any year. A person shall cease to be a member if their annual subscription remains unpaid at the 21^st October of each year.
A person, who becomes a new member between 1^st May and 30^th June in a membership year, shall have their membership automatically carried over to the next consecutive membership year.
The membership fee may be waived in cases of financial hardship at the discretion of the Board.
